You can send referrals with or without an account. Please use our secure online portal to refer your patients. I am a PRISM PATIENT with an EYE HEALTH concern and want to contact Prism FOR DOCTORS REFERRING PATIENTS Due to high call volumes, we recommend the online form for your convenience. If you are a Prism patient who wished to contact us with questions or concerns about your eye health, you can contact us using our secure online form. I would like to confirm or reschedule my appointment FOR PRISM PATIENTS WHO WANT TO CONTACT US If you are a Prism patient with an upcoming scheduled appointment and you would like to confirm or cancel the appointment, you can contact us using our secure online form. I am a NEW PRISM PATIENT and want to check in TO CONFIRM, CANCEL OR RESCHEDULE AN APPOINTMENT Instructions will also be emailed to you when you submit the form. Instructions for attending your visit are HERE and on the online check-in form. If you are a NEW PRISM PATIENT (I have not been to Prism before or I have a new referral) and have a confirmed in-person appointment at Prism, you MUST check-in online 4 to 7 days before attending your appointment. FOR NEW PRISM PATIENTS WITH A SCHEDULED APPOINTMENT Please note that if you are currently under the care of an optometrist, your request will not be processed. If you are not under the care of an optometrist, you can request an appointment with a Prism optometrist HERE If you do not have an optometrist, you can find one in your area on the College of Optometrists of Ontario Website. If you are a non-Prism patient who requires care, please see your optometrist or family doctor. PLEASE NOTE: With patient access to eye care services now increasing in Ontario, we are no longer accepting requests from the public for e-Assessments.
